cvsnt 1.11.1.3 build 57d

Restructured server code to remove the redundant thread.  This should
give a slight speed increase, and (more importantly) make the server
behaviour more predictable - hopefully the end of the 'end of file
from server' nightmare.
Changed script execution to work more like the Unix version.  This
won't break anything, but it might make the output from your scripts
look odd.  Previously cvsnt just passed the script output directly
back to the client, which meant you had to put protocol elements in
your output text (like 'E Bug identifier missing').  Now the protocol
is added automatically after the script.  This is how the unix version
behaves, and long term makes more sense.
Make cvs admin work remotely again.
Modified permissions code to check at file granularity.
Server now sends 'error ' after fatal errors.

I've tested this over the last few days, and it seems solid, however
the scale of the server changes mean it's possible something got
missed - wait a couple of days if your job depends on this working
properly :-)
